Care at Home companions offer companionship and engagement for you or a family member. They may accompany clients to social functions, medical appointments, movies, walks through the park and other activities. They can be a partner in playing cards or board games or reading to the visually impaired. They may also assist with homemaking activities.

Care at Home personal care assistants help clients with activities of daily living (ADLs). They provide competent hands-on care including help with:

  • Bathing
  • Dressing
  • Toileting
  • Ambulation

Your personal care assistant will also provide companionship and homemaking activities.

Care at Home’s live-in caregivers help clients with their daily routines over a scheduled period of time. We will match the live-in caregiver’s skill set to the level of care that you or a family member require.

Aging in Place

Is remaining in your current home a viable alternative for you or your family member? Aging in place means you are able to live in the home of your choice comfortably and as independently as possible. Starting with a conversation, we can work together towards your aging in place goals.

Planning meeting – We will focus on your family member’s specific circumstances to determine what is needed to gracefully and securely stay home, now and with potential future needs in mind.

Develop your aging in place plan – After we better understand your needs through an initial consultation meeting, we will develop a plan and can coordinate services that may include:

  • Initial suggestions for adapting the home to support mobility and safety
  • Personal care, assistance with household tasks, and an escort to community or other events
  • Referrals to experts in home organizing, home modification and services such as home security and medical alerts
  • Follow-up check-ins to help you and your family carry out your aging in place plan

Recuperation at Home

After a hospital or rehabilitation stay, returning home may pose short term limitations on your ability to manage personal and household needs. We customize our services so you can get home with the services you need to help you get back on your feet as quickly as possible.

Planning Meeting – Your Care at Home care coordinator will meet with you to determine and arrange assistance during your recuperation.

Our services may include:

  • Contact with discharge planners regarding post- discharge needs
  • Arranging for recommended items to promote your recuperation
  • Personal care, assistance with household tasks and errands and transportation

Circle of Care Bereavement Support*

When we lose a loved one, our emotions, including grief and loss, have an impact immediately and over time. There may be details requiring attention – some are easy, and some are painful. We are ready with emotional support, and information and guidance about practical matters. How we proceed together is based on your personal circumstances and needs.

Resources for support – Referral to Jewish Family Services for individual or group counseling to help you or family members work through this difficult process. Groups will be offered dependent on participant interest, circumstances.

Clean up and home re-organization – We can help with the often-daunting task of sorting through your loved one’s belongings. Care at Home will work with you at your own pace and support you during this process.

Plan for the future – When the time is right, we can:

  • Provide information and referrals regarding financial and legal planning
    • Coordinate personal care, assistance with household tasks, errands and transportation
    • Discuss options for staying in place vs. moving, and assist with the necessary planning

Stay Engaged

Living alone does not mean having to be alone. We are here if you or your family member is feeling isolated, or perhaps you are new to your community.

Planning meeting – We will work with you to help find meaningful activity to help you develop interests and become more connected.

Some examples of how our assistants and companions can work with you:

  • Visit in the home for socialization
  • Have lunch or dinner together
  • Help with activities such as using the computer, sorting mail and making appointments
  • Provide an escort for companionship and transportation to cultural events, errands, or personal services such as nail or hair care – Temporarily suspended due to COVID-19

We can also provide referrals to Jewish Family Services for individual or group counseling, as appropriate to your situation.

Planning meeting – We will meet with you or your family member to identify the products and services that make sense for each individual based on interests and abilities.

Get it done – our technology partners will work with you set up and teach you how to use common technology products and tools.

Care for the Family Caregiver

Caregiving for a friend or family member is rewarding, complex, and sometimes overwhelming. Care at Home provides education, support and respite so that you can assist the person you are caring for while taking care of yourself, too.

Planning meeting – We meet with you to discuss your circumstances and concerns. We will frame the caregiver role and how to manage the responsibilities of caregiving to help you be at your best while taking care of the person you care for.

Coordinate services for the caregiver – We will consider the care needs of the person receiving care and develop a care plan with you. In addition, we will:

  • Work with you on how to provide and schedule routine personal care for the person you are caring for
  • Arrange respite care from a few hours a week to more extensive hours if you need to be away
  • Refer you or the person you are caring for to Jewish Family Services to individual or group counseling
  • Provide information and referrals for topics such as home safety, financial and long-term planning, community resources, and the emotional impact of caregiving on you and your family

Jewish Holiday and Event Support

Remaining active in your community and with family and friends is important.

Holidays are a time for reflection, celebration and spending time with family and community. Holidays such as Passover, Yom Kippur and Rosh Hashanah also require preparation. If the thought of getting ready for the holidays is overwhelming, we can help you to continue with the traditions you have always followed.*

Planning meeting – Meet with us so that we can understand what is needed to remain active in the community, and how you prepare and celebrate holidays.

Navigating in the community – When attending events or services is a challenge, we will help. We can:

  • Provide an escort and transportation.
  • Contact the event site, synagogue or other destination to check on and arrange for any needed accommodations such as entry access or special seating

Holiday preparation – With your guidance, we can help prepare your home for holidays and events:

  • Provide the home making or arrange for a more comprehensive cleaning
  • Help prepare your favorite recipes or assist with arranging catering
  • For Pesach we can also:
    • Arrange housecleaning so that your home is “Kosher for Passover”
    • Help organize dishes and kitchenware for use during Pesach

* Care at Home can recommend cleaning contractors and food providers / caterers; clients will compensate these providers directly
